I don't really ever do. Do you think that if you don't keep one making gains is probably close to impossible if you don't really know what you're shooting for?

It won't keep you from making gains, but it sure will help with planning your workouts and finding what mix of volume intensity and frequency is optimal for you body.
Whenever I break a big PR, or suffer a set back, I look back at my log, and figure out what I was doing that caused it.

I feel it's good to have one just so you can look back and see where you were... if you're making any progress.

I do...I plan my workout ahead of time in a small notepad and keep it along with a pen in my gym bag (side pocket so it doesn't get sweaty). Then I know what I did the previous weeks, then I know when I reached all my reps at that weight and if I need to up my weight. That and I have a horrible memory of what I did the days or a week before, so I also do it to keep myself honest.

I never plan my workouts per se...only 'think about possible things I will do'....go for PR in 80% or 101%PR?....should i do this exercise or that?....then when i hit the gym knowing all the factors... I will lift by how I feel....enough sleep? ate well? how much time do i have? etc...lethargic or energetic? etc...
Journal helps to monitor progress and see what you have done previous workouts....especially if you're constantly changing your workouts...ala westside...or something else....journal is essential....just like a bottle of water during the workout....

i keep a journal so i know what i got last week and what to shoot for this week........it helps me know when to move up or down, what to cut out, what to add, etc......i also keep track of my calories, etc.........it will never hurt to keep track of every last detail, sometimes when you see your progress it keeps you goin......i luv it:)

When I go to do my workout tomorrow, I don't have the ability to remember exactly what I did in weight/sets/reps in my Wednesday regimen a week ago.
Workout log tells me where i've gotten to, and helps me get where I want to go.
It's not just a question of making gains. It's a safety factor too. One of the big reasons for weight-related injuries is moving up the weight too much at one time. A workout log helps me remember where I need to move up poundage, and the safe amount to increase it. Most people need to keep track -- I know I do.
