How can I lose weight for good?!?!

Hello everyone. For the past year I have exercised religiously at least 5 times every week (With very few exceptions.) Last year I started doing only 20 min on the elliptical, and worked my way up to between 60 and 90 minutes on either the elliptical, treadmill, stairstepper, bike or all three. (I am able to run 6 miles in under 60 minutes now if that counts for anything.) Also, I cut soda completely out of my diet, and rarely consume fruit juices. Although I have noticed some tone in my body, I have not lost any weight whatsoever. I have been the same height since 7th or 8th grade, but my weight has continuously increased. The lowest I can possibly remember myself weighing was between 115 and 120. Now I am 142lbs and the scale won't budge, unless it is going to increase. I DO NOT KNOW WHAT TO DO. I vary my cardio and I do strength training. I work my shoulders and chest on one day, bis and tris another day, back another day, and legs another day. (For each muscle group I usually do 3 exercises of moderate weight, 3 sets of 20 slow and controlled reps.) Also, as I said, I have been following this exercise regimine for exactly a year 5 times a week. As far as eating goes again, I am a vegetarian, I love eating japanese food and usually eat lots of vegetables, fruit, and lots of tofu and fish. WHAT AM I DOING WRONG and HOW can I possibly get down to an ideal weight (I'm thinking roughly 115lbs which would mean I have to lose 27-30 lbs) for my height of only 5 feet :-(
