What weight to start with?

If it says 12 reps, do I put enough weight on to just barley do the reps asked for without losing good form?

yep. and once you can complete the desired number of sets with good form, add a little more weight. It's progressive resistance.

Should I put on more to maybe miss 1 or 2 reps each time

Only add more when you can complete all sets of a movement with good form. The new weight should make you really work to complete a set, and maybe not even hit the full amount reps for the next set. You don't want to add too much, just enough to make the exercise a struggle.
