When I looked at my FT routine this week I had Lying Triceps Extension listed twice on my tricep/bicep day. Has this happened to anyone else. I assume that I should substitue one of them for a different tricep exersize.

sometimes that happens in the regular pro routines. There is nothing wrong with it as it is good to keep challenging the muscles with new things, eg. 8 sets of one exercise rather than 4.
In the ft PRO version, you are able to switch ANY exercise from a list of others. This makes it very customizable.
