"If you always do what you've always done, you'll always get what you've always gotten." You've probably heard that before, but I think it applies to weightlifting/bodybuilding more-so than many other aspects of life. I had this same problem until just a few months ago. Its possible that your triceps are fatigued by the time you get to incline. Try the pre-fatigue method on your chest.... Do flyes first so you don't involve your triceps. Then hit the incline bench next (I prefer dumbells for incline - better stretch, but its up to you), followed by flat bench. Keep changing the order of chest excersises and increase your intensity. It'll take time, but you will appreciate the results.

Those 2 are thew basics to building your chest . Reverse their order once in a while to mix it up

yeah I agree with jbennet. What you should be doing is working your weakest link first

you would be surprised how much strength decline presses can help put on. this lift is often overlooked but when i'm stuck in a rut i change my program so that i do decline first out of everything. you should give that a shot. also, make sure you're not pre-fatiguing your shoulders or triceps, whether it was from a workout a couple days ago, or earlier that day. that's an important one!
