Mine do that, ache during but not immediately after. However, by the morning, they are aching. Are yours not aching at all, or just not immediately? Try doing all your reps really slowly, that usually helps me. Or, try a completely different exercise for the same muscle.

Make sure that you are using good form and putting as much into it as you can and you will definitely feel it. Especially two days or so after the workout. It will let you know if you max'd out or not.

Alot depends on your level of training. After training for quite some time, at least intermediate stage, you will not necessarily have that..it is called DOMS(delayed onset muscle soreness). this does not necessarily mean you didn't have a good workout. If you pushed to the limit each and every set, and I don't mean just till you think you can't do another, but 2 more after that, and you "feel" it then, you have overloaded the muscle sufficiently. You may also need a change of workout, if you have been doing this workout for some time. Hitting the muscles from new "angles" with different exercises can bring that back to some areas.

are your muscles starting to ache or burn in the first set?

if it's not burning in the 1st set, maybe try lifting a weight that you can only do the # of reps you're supposed to do. i aim to feel the burn at the half way point for each set and almost to failure on the last rep and set.
doing diff exercises and doing them slower with good form is good advice too.
