Well, last week when I finished my 3 rep (135lbs) on bbell incline press I had complete muscle failure (fatigue) because I didn't do my warm up set low enough. I didn't have a spotter and had to let the weight roll slowly down to the floor as I couldn't tip it. Luckily it was only 135 as I am 170.
Goes to show though from the article we are all at risk.
Smith machine works well in place of a spotter.
