I am facing health consequences due to the neglect I have shown my body. At 5'6" and almost 300 pounds, I have trouble doing anything. I can't say how much I hate myself but I take responsiblity for how I got in this shape. I could use any advice and suggestions as I make healthier changes in my life.

Some small changes I have made are:
1.Changed white bread to wheat bread
2.Cut my diet pop in half and moved to water
3.Replaced real butter with olive oil butter
4.Try to stay away from fast food

First off get your doctors ok to start exercising, and clean up your diet.
Nice to see you want to change to a healthy lifestyle. there are some good programs on here to help you out, along with a good bunch of people.

The first step is taking responsibility, the second is making those changes, one step at a time is easiest. As DaniDIEt does, and if it is easier for you, start by making some small changes in your diet, start cutting back on portions size, cut out sodas, etc. Then put one foot in front of the other and start walking, that is the easiest form of exercise and is easiest to monitor. Match your breathing to your walking, in other words, pay attention to your body, to your heart beat, to your breathing. If you get out of breath, slow it down, challenge yourself without going over your limits.
You are here and that is a great start, you have taken responsibility and you have asked for help - you are on the right track, just keep onward and forward and you will succeed if that is what you want!! :)
