When doing HIIT on a treadmill, I usually do 5 minutes warmup, 30 seconds on, 60 seconds off, for 8 cycles, then 5 minute cool down, for a total of 22 minutes. Is this right? Or am I supposed to gradually go up at each high point then go back down, back up higher, etc,

It depends.
Make sure you are challenging yourself in your max. and make sure your are giving yourself enough time to recover to pull off the trick on your heart.
I may consider extending the times a bit, but again - it's up to you. The key is to keep your body guessing and to challenge yourself. Your last sprint should be your hardest.
Here is an example of what I have found works for me well:
Cardio 33 Mins (3 Miles in 27:23)

3:45 @ 5.5 mph, 1:30 @ 8.6 mph
3:30 @ 5.6 mph, 1:30 @ 8.8 mph
3:30 @ 5.7 mph, 1:30 @ 8.8 mph
3:30 @ 5.8 mph, 1:30 @ 8.9 mph
3:30 @ 5.9 mph, 1:30 @ 9.0 mph
3:30 @ 6.0 mph, 1:30 @ 9.1 mph
2:45 @ 3.5 mph
