I had a deppression late in life although not as severe as yours sounds. Prescription from your doctor has got to be your first start and to talk about and identify if that is for life or as an aid with a view to stop the pills given. I strongly believe exercise controls my deppression, I no longer am on seratonin inhibitors, sleeping pills etc... Hey they weren't a bad thing, just a temporary aid- you may need a permanant programme or not- you need a medic who cares. Anyway exercise...
I find a very fast set of interval training on an eliptical or bike brilliant. You have to get your heart rate to a point where you cant f##ing breath no more, on the verge of your maximum ever heart rate bpm! Trust me, at that point you CANNOT stop smiling! Search the web for bipolar and deppression and net doctor forums for cognitive therapy.. These exercises work, you must feel you really want a go at it like these;
squeeze with thumb and finger the skin/palm between the right hand thumb and forefinger for a minute- it will relax you.. pout your stomack out and hold- it releases pressure on your liver and internals thus relaxing you a little.
close your eyes and imagine you are surrounded by purple light and you are safe and content- this should give you a feelingg of protection, security etc... it's calming.
