Before you stand should you're back be at a 90 degree angle with your thighs? I find I need to lean forward a tiny bit before standing or I lose balance.

Nope. If you can sit at 90 degrees on a below parallel box squat, then you are not sitting back far enough.

Sit back and down onto the box, relax your hip flexors for a second, and then explode back up, starting from the same position that you descended in.
